** In Order to ensure that your child receives
his/her T-shirt during Vacation Bible School week, this form must be
completed and submitted no later than June
15th.
Students submitting forms after June 15th will
receive their T-Shirts as soon as they arrive. Shirts arriving
after VBS ends will be mailed to students.

Because we serve refreshments of punch & cookies to the children
each day, please note any food allergies your child(ren) has so we may be

Permission to attend picnic
My children, named above, have my permission to attend the
Massillon Church of Christ Vacation Bible School picnic, which will be
at the Kiwanis' Park on Oak Avenue NE in Massillon on Friday,
June 27th. I give my permission that transportation to and from
the park be done by car.
2 and 3 year olds must be accompanied to the picnic by an adult.
On Friday, your child can be picked up at the church building
between 12:45 and 1:00pm.
